Connecting the Pieces is seeking a Service Coordinator.

We offer comprehensive services to students who present with challenging behaviors and educational need. CTP provides individual attention to each student we serve while bringing all those working with the student together to ensure a collaborative effort in helping the student reach their fullest potential.
Responsibilities:
- Manage all aspects of a student caseload
- Ensure students receive their Related Services as outlined in their Individualized Education Program (IEP)
- Initiate, monitor, update, and ensure the completion and documentation of services for existing and new students
- Track the service delivery of providers assigned to students within caseload
- Respond to and problem solve all issues related to the delivery of services to students on caseload
Job Duties:
- Communicate and document key information relating to students’ services to appropriate CTP staff, providers, and school representatives
- Accurately track the utilization of authorized student related services
- Document all service issues and exceptions in a timely manner and in sufficient detail
- Monitor provider and client attendance, absenteeism and cancellations
- Provide exceptional customer service and support to staff, providers, and schools
- Proactively resolve problems and issues
- Participate in provider and peer audits
- Assist in student referral life cycle
- Assist the Student Services, Payroll and Billing teams with ensuring that contractors submit appropriate documentation of services provided
- Ensure strong contractor relationships
- Assist the Student Services team in the reconciliation of client service discrepancies
- Proactively demonstrate commitment to and support of team activities, projects, assignments, and responsibilities
- Participate in team and organization meetings, events, in-services, and trainings
- Fill in for other service coordinators in their absence
- Other duties as assigned
Qualifications/Characteristics:
- Extreme attention to detail
- Strong ability to multi-task and operate in situations with regularly changing priorities
- Exceptional time management skills
- Excellent interpersonal skills, both written and verbal
- Ability to build and maintain good relationships with various groups of people both internal and external to CTP
- Demonstrated working knowledge of MS Applications, especially MS Excel
- Collaborative, team player who can also work independently
- Ability to learn and retain new tasks, systems, and processes quickly
- Ability to excel in a fast-paced environment
- Ability to effectively prioritize tasks and responsibilities
- Ability to problem solve and tackle issues to their resolution
- Associate’s degree preferred
- Previous experience in a coordination position preferred

Salary: starting at $35,000/yr.-$40,000/yr.
